SQ UM waiver form

im travellig alone on singapore airlines to chennai on 30th April 2016.im currently 17 years old and i have to present the UM waiver form during check in.it asks for a witness.only my mother would be coming to drop me at airport as my dad is out of town.who should sign as the witness?or can i leave it blank?

Ask your mom to accompany you to the check-in desk and sign it in the presence of the staff. The witness field is supposed to be signed/stamped by the ground staff at the airport.

In my experience, the form needs signing just by the pax and by a parent/ carer. I've travelled as UM and checked in without any parents/ guardians present, and simply handed the signed forms.
