    A Qatar Airways Boeing 777-300ER, registration A7-BAC performing flight QR-778 from Miami,FL (USA) to Doha (Qatar), lined up runway 09 at taxiway T1 (approximately 2600 meters/8500 feet takeoff distance available, full runway length 3968 meters/13,016 feet) and departed Miami's runway 09 but struck the approach lights runway 27 during departure. Both tower, departure controllers as well as crew maintained routine communication. The aircraft continued to destination for a landing without further incident about 13.5 hours later.

    On Sep 17th 2015 the FAA reported the aircraft struck approach lights on departure from Miami and continued to destination. The aircraft received substantial damage to its belly, the occurrence was rated an accident.

    Seems like a huge mistake that could have easily turned into catastrophe. These were the runway 27 approach lights:



    Three poles completely, and one partially missing their approach lights. Seems incredibly foolish to load up a 77W for a 13 hour flight and commence an intersection take-off. Shades of SQ286 in 2003 at AKL. Official report from QR stated that there was damage to the belly, so it's quite a wonder pilots didn't return to MIA. Biggest issue here seems to be that they continued the flight - what if damage had been more severe? Another JAL 123?

    No pictures yet, Qatari press is more restricted than ours , so we'll probably only get to see after NTSB releases their report.

    I wonder what truly happened. Incorrect entering of load (a la SQ286) leading to EFB calculations suggesting T1 intersection take-off was sufficient? Or pilots forgetting that they weren't at the full length of the runway? But RAAS should have informed them either way. Surely the flight crew knew that aircraft was either slow to accelerate, or was using up a hell of a lot of real estate before V1/Vr. If they did not know that the aircraft struck runway lights, then surely cabin crew knew, even if it was a 2033hrs departure? After all it's not just the gear but the belly that scraped the lights.

    Big mistake that could have killed many, many people. Let's see how this turns out.

    I ran the numbers through our preformance computer and the best I could get is a 37,000 Kg penalty for using T1 instead of the full length. For a MIA-DOH flight, I don't see how a departure from T1 could have been done legally.

    They were very very lucky. Not only that they got the boat airborne, but that the damage incurred didn't land them in the Atlantic after!
